What is a Family Health Team?

A Family Health Team is a group of health care providers who work together to help keep you and your family healthy and to coordinate the best possible care for your health needs. A team approach offers you better access to appropriate health care in your own community.


What health professionals are part of the KDFHT?

The Kirkland District Family Health Team includes registered nurses, nurse practitioners, dietitians, Respiratory Therapists, Pharmacist and social workers. These interdisciplinary health professionals have expertise in specific areas and help support the efforts of your doctor in caring for your health.

 

How can I see a KDFHT health professional?

You must be a registered patient of one of our participating family health networks Physicians and referred by one of our participating physicians or Nurse Practitioner. Your physician or Nurse practitioner will let you know how to book an appointment.

 

How much will this cost me?

There is no cost to you for any of the services. All the programs and services offered by the Family Health Team are funded by the Ontario Ministry of Health and Long-Term Care.

 

Will I always see my family physician at my appointments?

Not necessarily. Sometimes you may see another member of our interdisciplinary team.
